Everything went wrong for McCaffrey in 2024, as what was called a calf strain in early August ended up being Achilles tendinitis in both legs and delayed his season debut until Week 10. The 49ers had suggested he was on track to play Week 1, right up until gameday, yet they denied any setbacks had occurred even after he went on injured reserve. Whatever the case, McCaffrey was not quite himself upon his Week 10 return, immediately handling snap shares above 80 percent but averaging just 3.5 YPC over his first three games. He got off to a hot start in his fourth game, Week 14 at Buffalo, only to suffer a season-ending PCL sprain in the second quarter. He once again was all-or-nothing in terms of durability/injury luck, playing fewer than eight games for a third time in the past five years (whereas his other five NFL seasons all featured 16 or 17 regular-season appearances). He said in early January that he was almost back to full strength, having avoided surgery in the offseason. San Francisco's offensive line is a slight concern with guard Aaron Banks off to Green Bay and left tackle Trent Williams turning 37 in July, but it's generally a poor bet to fade a Kyle Shanahan offense, and McCaffrey is still (barely) on the right side of 30 and just two years removed from his 2023 career highs for rushing yards (1,459) and YPC (5.4). Still, McCaffrey was running on the side field Friday and told David Lombardi of SFStandard.com that he "feels great physically" and plans to play through the calf injury, which he described as "minor." While McCaffrey's status for Week 1 may not be made official until 90 minutes ahead of Sunday's 4:05 p.m. ET kickoff, it sounds like he'll take the field barring an unexpected development during pre-game warmups.

McCaffrey was the consensus RB1 and first fantasy pick from 2019 to 2021, but injuries in 2020 and 2021 left his ranking up for review after two dud years. That didn't last long. Not only has McCaffrey's health cooperated the last two years, he now has the privilege of playing in a top-three offense. Even with sky-high usage the last two years (516 carries, 152 receptions), McCaffrey's efficiency was at/near a league-leading level (5.0 yards per carry, 6.8 yards per target). Volume is normally a slight downward pressure on efficiency -- the more a defense expects you, the better they tend to defend you -- but McCaffrey has been unstoppable in San Francisco no matter how aggressively opponents try to slow him. While his previous injury troubles raise the understandable fear of similar problems occurring in the future, McCaffrey's overall track record of availability (79 percent of possible games) is about normal for a heavily used RB. Rather than fretting over McCaffrey's past injuries, the more useful frame would be to consider that injuries are likely the only way for any other running back to catch him in the fantasy rankings. That's a statement that would apply to very few running backs throughout history, as a talent like McCaffrey in a scheme like San Francisco's yields an almost perfect projection.
